This Lock Out/Tag Out (LOTO) Logbook provides a systematic record of machinery and equipment isolations, and their controlled removal, in accordance with the International Safety Management (ISM) Code and the company Safety Management System (SMS).
Systematic use of this Lock Out/Tag Out Logbook enables the Master, Chief Engineer and responsible officers to:
- Provide a documented audit trail of all energy isolations and their controlled removal in support of the ship's SMS
- Meet MLC, 2006, Regulation 4.3 requirements for occupational safety and health protection and accident prevention
- Ensure traceability between isolation events and their corresponding permits-to-work
- Demonstrate compliance with ISM Code Sections 7, 10 and 11 during flag state surveys, port state control inspections and ISM audits.
